A sworn declaration is a document containing statements that you declare and affirm to be true, similar to an affidavit. This must be done in front of a commissioner of oaths, who is authorized by the Province of Ontario to witness certain types of sworn declarations. You may be required to affirm a declaration when applying to the government for various immigration options or if a company needs specific information from you. This could involve filling out a form provided to you or writing the declaration yourself.


In some cases, government forms require you to sign them in front of a notary public. A notary public is an individual authorized by the Province of Ontario to witness the swearing of oaths and certify documents. If your declarations are to be submitted outside of Ontario, they will need to be notarized. You might also seek notary services if you require certified true copies of important documents. We can help by making photocopies and then certifying them as copies of the original with the notary public's signature.


Examples of declarations include statutory declarations regarding common law status, marriage abroad, work experience, invitation letters for visit visas for family and friends, and certified true copies of various documents such as driver's licenses, passports, diplomas, birth certificates, and permanent residency cards. Other forms could include a travel consent letter for children traveling abroad with a spouse or family member, a custodianship declaration form, passport applications for adults or children, name change applications, sworn statements for family gifts of used vehicles in the Province of Ontario, guarantor forms, OSAP forms, used vehicle transfer forms, common-law declarations, automobile proof of loss insurance forms, certificates of authorization for professional corporations, diplomas and degrees, and affidavits, including those involving a commissioner of oaths. Additionally, we can assist with accident benefits claims and ensuring all your documents are properly certified.
